Official report filed at the police registration office, Polizeipräsident, in Berlin when Heinrich Loewenstein [Henry Lowenstein] emigrated to England. The form has been filled out by hand and signed by Dr. Max Loewenstein. Dated June 5, 1939, the day Heinrich left Germany, the top of the form gives his former address in Berlin and his destination as London, England. Information columns on the form are filled out for Heinrich and include: surname, first name, marital status, profession, birth date, birthplace, nationality, religion, and military service. Two official stamps, one in the top right corner with the address of a police station and date, and an official Nazi stamp left of center of the sheet. The bottom of the form has been signed by an administrator and is dated June 8, 1939. The back of the form has extracts from the Reich Registration Regulations.
